CMR OrbitGlobal Medical Training Platform

Turning cardiac MRI education into an interactive, browser-native experience. We developed the full platform for the Circle Institute — from a custom DICOM viewer and gamified course architecture to the AWS infrastructure underneath — so that cardiologists and radiologists worldwide can train on real scan data and progress toward SCMR, DGK, and EACVI certification without ever installing software.

Client: Circle Institute

Together with: Julien Leske, the Circle Institute medical team, and Level-III CMR instructors

Goal: A visionary e-learning environment where medical professionals can analyse real cardiac MRI studies, complete structured case bundles, and earn certification credits — entirely in the browser, on any device.

How we got there: We covered the entire technical scope: product design, Next.js frontend with gamification elements, Node.js backend connected to two OrthanC DICOM servers, and a robust AWS infrastructure with load-balanced EC2 instances, CloudFront CDN, and S3 storage. The custom DICOM viewer was the centrepiece - it had to render complex medical imaging data reliably while meeting strict certification-body requirements. Tight feedback loops with the medical team ensured every clinical workflow made sense in its digital form.

CMR Orbit sits at an unusual intersection of engineering rigour and clinical sensitivity. Generic LMS solutions couldn't handle the specifics — in-browser DICOM rendering, structured course progression with quiz modules, and GDPR-compliant infrastructure for medical data. So we built it from scratch. The gamified course architecture keeps learners motivated through constellations and case bundles, while the underlying infrastructure handles the heavy lifting of serving high-resolution scan data across continents. Since launch in early 2024 the platform has been adopted by physicians in multiple countries, confirming that thoughtful product engineering can genuinely move a medical discipline forward.
